Enrollment - Returning Student

Welcome back to a new year of learning in MSAD 58.  Each year we update our student information system.  Here are the forms that are completed each year for a child in school.
 
If you prefer printed materials here are the documents we will need completed each year.  
  1. Annual Information Update (please note this will be sent home with your child)
  2. MSAD 58 Annual Student Health Update  (MSAD 58 Concussion information can be found here)
  3. MSAD 58 Permission to Publish
We encourage all families to complete an application for Free/Reduced Lunch status.  All meals are free again in 2023-2024, but the information from the forms is essential for our ability to get grant programs to support learning and wellness in the district.
